How do I add images to a FAQ entry?

You can use the bundled WYSIWYG Editor with the Image Manager to upload images or link images from other sites. phpMyFAQ supports all kinds of images, the PDF export supports JPEG and PNG images, but no GIF images.

I am not seeing any buttons or controls in the wysiwyg editor that is for attachments. What am I missing? I am running v2.6.4.
